Required Event Forms

• 4-H Enrollment Card
  A 4-H member must complete this card each year to be a Lowndes County 4-H member.
• 4-H Code of Conduct Form
  A 4-H member needs to complete this form once during the enrollment year (August - July).
• Medical Information & Release Form
  A 4-H members must complete this form for every out of town 4-H event as well as some local 4-H events.
• Fall Forum Registration Form
• National 4-H Conference Information and Link for Application
