Membership Information

Membership Requirements

4-H is the youth development club of The University of Georgia Cooperative Extension. Membership is free and open to all youth ages 9-19. Some specialized 4-H programs are designed for certain age groups based on grade eligibility.

The only form required to join this club is a 4-H Enrollment Form (below).Simply complete this form and take it with you to the next club meeting. Check out the club(s) to see which may be of interest to you/your child.

Required Forms

• 4-H Enrollment Card (pdf)
  A 4-H member must complete this card each year to be a Lowndes County 4-H member.
• 4-H Code of Conduct Form
  A 4-H member needs to complete this form once during the enrollment year (August 1 - July 31).
• Medical Information & Release Form
  All 4-H members must complete this form for every out of town 4-H event as well as some local 4-H events.
