Medical Information and Release Form
The Medical Information and Release Form is required each time a 4-H'er participates in an event. Please print and complete both pages of the form. 4-H'ers taking ANY type of medication, both over the counter and prescription medication, must complete the Medication Summary side of the form.

4-H Code of Conduct
The Code of Conduct must be filled out for youth to participate in 4-H activities. Both the 4-H'er and the parent/guardian need to sign this form.

Fall Form for Senior 4-H members will be held at Rock Eagle on December 10-11. For more information go to: georgia4h.org
