- Medical Release Form & Code of Conduct Form are required for each activity.
- Interested in being an adult chaperone? Effective January 1, 2009, all volunteers must be certified, which includes a background check. Complete the Volunteer Application form. Then give us a call to discuss the required training!

Putnam County Archery Team meets every Tuesday from 6:30-7:30 at the 4-H office. ! Call the 4-H office today to get in touch with Mr. Mitts our certified archery lead coach!

General Info
We are located in the old National Guard Armory Building, 663 Godfrey Highway, Suite 101. Our office hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. We are closed from noon till 1:00 p.m. for lunch.
