Project Achievement

  • Your Project Achievement form - this is a useful tool to help you get started
  • Competition will take place April 17, 2010 in Douglasville, GA. 4-H'ers participating will meet the school bus at Orrs Elementary parking lot. They will travel with our certified/screened adult leaders to Douglasville for the competition. Parents may not ride the bus but may follow the bus to Douglasville. Parents are not allowed in the classroom during the competition. They may wait in the cafeteria for the awards program when all 4-H'ers have finished their demonstrations.

Enrollment Cards

If you attend a private school or are home schooled and haven't completed an enrollment card, you will need to provide the information on the enrollment card (link below) or email the 4-H office at uge2255@uga.edu with the information listed below.

Download the enrollment card, fill it out, and email it to the 4-H office. If you don't have the capability of saving the card to your computer, filling it out and emailing it, you can send a regular email to the 4-H office and include the following information:

  • Your name
  • School name
  • Grade
  • Your complete birthdate
  • Your complete address including apartment number, city, and zip code
  • Home phone number
  • Your email address so we can send meeting reminders each month
  • Race - White, African American, Asian, American Indian, Hispanic
  • Gender - Male or Female
  • Is your mom or dad in the military right now?
  • Dad's name - first and last
  • Mom's name - first and last
  • Parents' email address
