  • There were 102 participants in the Cloverleaf Project Achievement in Douglasville on Saturday, April 14. The results of that competition for Spalding County:
    • 37 First Place winners
    • 23 Second Place winners
    • 17 Third Place winners
    • Highest percentage of winners
    • largest number of participants
    • highest percentage of increased participants

Enrollment Cards

If you attend a private school or are home schooled and haven't completed an enrollment card, you will need to provide the information on the enrollment card (link below) or email the 4-H office with the information listed below.

Download the enrollment card, fill it out, and email it to the 4-H office. If you don't have the capability of saving the card to your computer, filling it out and emailing it, you can send a regular email to the 4-H office and include the following information:

  • Your name
  • School name
  • Grade
  • Your complete birthdate
  • Your complete address including apartment number, city, and zip code
  • Home phone number
  • Your email address so we can send meeting reminders each month
  • Race - White, African American, Asian, American Indian, Hispanic
  • Gender - Male or Female
  • Is your mom or dad in the military right now?
  • Dad's name - first and last
  • Mom's name - first and last
  • Parents' email address
