Enrollment Cards
If you attend a private school or are home schooled and haven't completed an enrollment card, you will need to provide the information on the enrollment card (link below) or email the 4-H office with the information listed below.
Download the enrollment card, fill it out, and email it to the 4-H office. If you don't have the capability of saving the card to your computer, filling it out and emailing it, you can send a regular email to the 4-H office and include the following information:
- Your name
- School name
- Grade
- Your complete birthdate
- Your complete address including apartment number, city, and zip code
- Home phone number
- Your email address so we can send meeting reminders each month
- Race - White, African American, Asian, American Indian, Hispanic
- Gender - Male or Female
- Is your mom or dad in the military right now?
- Dad's name - first and last
- Mom's name - first and last
- Parents' email address
