Important Forms

  • 4-H Enrollment – Completing a 4-H Enrollment Card is required to participate in Sumter County 4-H programs.
  • Code of Conduct – This form must be completed annually by all active 4-H members and returned to the office.
  • Medical Release Form – This form is required to each and every 4-H event you attend.

Get ready for the Holiday Season! Sweet potatoes will be delivered before Thanksgiving.
Purchase 40 lb. box for $25 from Sumter County 4-H Club.
