- 4-H Enrollment – Completing a 4-H Enrollment Card is required to participate in Sumter County 4-H programs.
- Code of Conduct – This form must be completed annually by all active 4-H members and returned to the office.
- Medical Release Form – This form is required to each and every 4-H event you attend.
