Workers Compensation
Note: If the injured employee receives a check from UGA the CEC must file for Workers Compensation through the CAES Business Office, not the county.
Procedures to Follow:
- If an employee suffers an injury while performing their duties, the injury should immediately be reported to the supervisor.
- For life-threatening Injury:
Seek treatment from the nearest hospital emergency room.
Supervisor should complete the following forms and fax to Jessica Kirk at 706-503-0236.
- First Report of Inury Form
- Wage Statement Form (when out of work more than 2 days)
- If not life threatening injury:
Wait to go to a doctor until one has been assigned to you through the Business Office. Supervisor should complete First Report of Injury Form and fax to Jessica Kirk in the CAES Business Office at 706-503-0236.
Jessica will call Amerisys and report the injury. She will call you back with a Workers Compensation number and Contact Name # to set up an appointment with a local doctor.
