Forms, Checklists, and Other Documents

New Hire Checklists

District Offices should please submit the following form to the Extension OrganizationalDevelopment Office extodev@uga.edu for processing of new employees. Forms should be submit as early as possible to allow for registration of particpants in required Foundations Trainings.

Please be sure to note any special training requirements or exemption from any required sessions.

  • County Extension Associates New Hire/Transfer (pdf)
  • County Extension Agents, Extension Specialist, and County Program Assistants New Hire/Transfer(pdf)

 

Progress Reports

Your County Extension Coordinator is required to complete three progress report forms after you have been employed for three, six, and nine months.

The CEC should meet with you within 30 days of those periods to discuss:

• work experience and progress to date
• any concerns
• major strengths and areas needing improvement
• establishment of goals
• progress made toward meeting goals.

You and the CEC should keep copies of completed reports. The CEC will forward a copy to the District Extension Director.
