Communication

Participation Waivers
Waivers of liability are necessary if you, as an Extension employee, will be leading a non-Extension group in physical activity. This includes any sort of group activity that you may lead that includes physical activity (kick-off event, senior center activity, coordinating your team with non-Extension employee members, etc.). If you will be leading a group, collect the following waiver and release form from all adult participants and keep it on file at your local office. If you are also offering a Walk-a-Weigh program, use the waiver and release forms in that curriculum instead for those individuals participating in that program.
If you are not personally leading or exercising with a non-Extension group doing physical activity, you do not need to collect these waivers. This is to protect you, not to make extra work.
Photo Release
If you take photographs of any non-UGA employees for the sake of this program, please have subjects sign the following form. This form should be printed on Extension letterhead. Keep all signed forms on file at your office.

Participant problems with registration?
Do your participants need help with registration? Send them to the following link to view a short presentation on registering.
If you need to demonstrate the registration process to large groups, download either of the following presentations for off-line viewing. This is a stand-alone presentation that will not require additional software.
